Individualized Care

We provide our individuals with a network of resources and provide quality behavioral health care. As a behavioral health collective, we assist members by providing therapeutic methodologies, individual approaches, goal structured management, coaching, and more. We help our clients connect with their community and develop an individual plan of care that can meet their needs.

It’s essential for us that our individuals get personalized approaches from our team to best match individualized goals. Rather than doing things the typical way, we prefer to adopt a system designed around your needs.

Our Services

Eligibility:

  • Vintage Solutions serves: Cabarrus County, Catawba County, Gaston County, Mecklenburg County, Union County.

  • NC Innovations Waiver Services: A referral from the Managed Care Organization (MCO) that serves the individual is needed. VS staff can assist with obtaining referral. For B3 Respite: The individual or family will need to provide Vintage Solutions with a current psychological evaluation. Screening required.

  • This program accepts Medicaid.

 

ALTERNATIVE FAMILY LIVING

 

NC Innovations Residential Supports: Residential Supports Level 1-4 (AFL) services are designed to assist individuals in acquiring, improving and retaining skills in self-help, general household management, meal preparation, personal finance management, socialization, other adaptive areas and assistance in activities of daily living. These services are provided in an alternative family living home and provide assistance, support, supervision and monitoring to allow the individual to participate in home life and community activities.

 

COMMUNITY NETWORKING

 

NC Innovations Community Networking: Community Networking (individual or group) is a service that provides individualized day activities that support the individual’s definition of a meaningful day in an integrated community setting. A goal of this service is to be inclusive of individuals with disabilities within the community, therefore it is provided separate and apart from the home of the individual and that of the provider. 

 

COMMUNITY LIVING SUPPORT

 

NC Innovations Community Living and Support (Individual or Group): This service is intended to increase or maintain the individual’s life skills or provide supervision needed to maximize self-sufficiency, increase self-determination and participate in their community. This service also provides the opportunity for unpaid supports to receive technical assistance that will help them ensure that achieved goals are practiced and maintained.

 

SUPPORTED EMPLOYMENT

 

NC Innovations Supported Employment: Provides assistance to adults 16 years or older with choosing, acquiring and maintaining a job. Initial SE assists individuals with developing skills to seek, obtain and maintain competitive employment or develop and operate a microenterprise. Employment positions are found based on individual preferences, strengths and experiences. The transition to long term SE should occur within one year of successful competitive employment as the individual becomes more independent in his or her job duties. SE services occur in integrated environments with non-disabled individuals or is a microenterprise owned by the individual.

 

RESPITE

 

NC Innovations Respite: Respite (individual or group) provides periodic support and relief to the primary caregiver(s) from the responsibility and stress of caring for the individual. It may include in and out of home services, overnight service, weekend or emergency care. Respite can be provided in the individual’s home or in an out-of-home setting.
